The Meal Choice Order Form gives Meals on Wheels clients the opportunity to select the meals they would like to receive. Choose from a variety of nutritious options designed to support your health, preferences and lifestyle, all while enjoying the convenience of Meals on Wheels.

How to Complete the Meal Choice Order Form

Your total meal selections must not exceed how many meals you receive each week. If you don’t know how many meals you receive each week, call (231) 755-0434, Option 1.
You may select more than one of the same meal, but your total selections must not exceed how many meals you receive each week.
Start by choosing the menu that matches the type of meals you would like to receive.
Enter the quantity of each meal you would like in the box next to the meal selection.
Click “Place Order” at the bottom of the form. We’ll follow up if we have any questions.

Don’t want to submit your order form online?

No problem! You can return the paper copy you received from your delivery driver back with your driver, mail to 275 W Clay Ave Ste 100, Muskegon, MI 49440 or call (231) 755-0434, Option 1.

When should you re-submit your order form?

After your initial submission to start choice meals, you only need to re-submit when you have changes to your order. Changes can take up to 10 business days. You can submit changes by re-submitting an order form online, with your delivery driver, mail or phone.
